NUESTROS MAYORES
Su gran descubrimiento marca un jalón
en el mundo científico argentino, y es de
una trascendencia innegable en la vida
del mundo: la adición de citrato de sodio
para efectuar transfusiones sanguíneas.
En efecto, el 9 de noviembre de 1914, en
una sala del Instituto Modelo de Clínica
Médica, tuvo efecto la primera aplicación
del método para reponer el caudal de
sangre perdido por un paciente, cuya
anemia. provocada por una hemorragia
nasal repetida, ponía su existencia en
inminente peligro.
La concentración de citrato de sodio
utilizada como anticoagulante es una
variable preanalítica importante porque
puede variar el tiempo de coagulación
del plasma sanguíneo ya que la cantidad
de citrato presente afecta la
concentración de calcio utilizada en
estas pruebas

La antitrombina es
una pequeña
molécula que
desactiva varias
enzimas de la
coagulación. La
afinidad por éstas
(su efectividad) está
potenciada por la
heparina.
Acelerando la
acción de la
antitrombina III en
1000 veces.
En la insuficiencia renal (especialmente en
el síndrome nefrótico), la antitrombina se
pierde en la orina, lo cual lleva a una mayor
actividad del Factor II y del Factor X, y a
una marcada propensión a la trombosis.
La heparina clásica ejerce su efecto
anticoagulante acelerando la
formación de complejos moleculares
entre la antitrombina III y los
factores II (trombina), IX, X, XI y
XII, que quedan inactivados. Tiene
particular importancia la acción
ejercida sobre la trombina y el factor
X.

Se encuentra naturalmente en
pulmones, hígado, piel y células
cebadas (mastocitos).

La heparina tiene una gran afinidad por el
factor 4 plaquetario (PF4), una proteína
tetramérica cargada positivamente que se
encuentra en los gránulos alfa de las plaquetas
y en la superficie de algunas células como las
endoteliales y las plaquetas. Cuando la
heparina y el PF4 se unen forman un complejo
heparina-PF4 que sufre un cambio
conformacional y expone nuevos epítopos, que
actúan como inmunógenos

Among patients with reduced LVEF who
were in sinus rhythm, there was no
significant overall difference in the primary
outcome between treatment with warfarin
and treatment with aspirin. A reduced risk
of ischemic stroke with warfarin was offset by
an increased risk of major hemorrhage. The
choice between warfarin and aspirin should be

The selection of an anticoagulant agent (warfarin, dabigatran,
apixaban, or rivaroxaban) for permanent/persistent/paroxysmal
AF should be individualized on the basis of risk factors, cost,
tolerability, patient preference, potential for drug interactions,
and other clinical characteristics, including time in the
international normalized rate therapeutic ration if the patient has
been taking warfarin.
Chronic anticoagulation is reasonable for patients with chronic
HF who have permanent/persistent/paroxysmal AF but are
without an additional risk factor for cardioembolic stroke (in the
absence of contraindications to anticoagulation).

Pharmacological Treatment for
Stage C HFrEF (cont.)
Anticoagulation is not recommended in patients
with chronic HFrEF without AF, a prior
thromboembolic event, or a cardioembolic source.
Statins are not beneficial as adjunctive therapy when prescribed
solely for the diagnosis of HF in the absence of other indications
for their use.
Omega-3 polyunsaturated fatty acid (PUFA) supplementation is
reasonable to use as adjunctive therapy in patients with NYHA
class II-IV symptoms and HFrEF or HFpEF, unless
contraindicated, to reduce mortality and cardiovascular
hospitalizations.

Tienen algunas ventajas importantes:
1) la posibilidad de su administración en una sola

Algunos estudios indican superioridad frente a la
HNF, especialmente en los enfermos de mayor
riesgo.
Su inconveniente más importante es su costo
más elevado.

estrategia farmacológica es preferible
enoxaparina a heparinaNa52 (clase: IIa/NE:B)1,2.

En caso de insuficiencia renal ajustar dosis
según ClCr.
En caso de ClCr inferior a 30ml/min, esta´
contraindicado su empleo.

Si se elige la estrategia intervencionista , puede

puede usarse en el momento de realizar la
intervencion, en vez de IIb/IIIa, si esta medicación no
se había prescrito inicialmente (clase: IIa/NE:B).
Si tras el cateterismo el paciente debe recibir
tratamiento quirúrgico , debe suspenderse 3 h antes
de la cirugía.

una evidencia clase: IIa/NE:A, excepto
si se ha de realizar cirugía de
revascularización en las primeras 24
hs y en pacientes con insuficiencia
renal grave.

(tenecteplase, tissue plasminogen activator,

physician’s choice. All patients received aspirin.

La HEPARINA NO FRACCIONADA
disminuye fenómenos de retrombosis
al inhibir la trombina circulante, pero no actúa
sobre la trombina ligada al trombo; impide la
agregación plaquetaria y de la fibrina ligada al
trombo, con lo que se evita su crecimiento y permite
que la fibrinólisis endógena
actúe in vivo
La dosis se ajusta al peso del paciente, de 80 U/kg como bolo
inicial, y continúa con una infusión a razón de 18 U/kg/h. La
dosis debe modificarse de acuerdo con el tiempo parcial de
tromboplastina (TPT); el objetivo es mantener entre 1.5 y 2.5
veces el control. El monitoreodel TPT debe realizarse cada 4-
6 horas,

to 325 mg once daily) (Grade 2B) or combination therapy with
aspirin and clopidogrel (Grade 2B). For patients who are
unsuitable for or choose not to take an oral anticoagulant (for
reasons other than concerns about major bleeding), we suggest
combination therapy with aspirin and clopidogrel rather than
aspirin (75 mg to 325 mg once daily) (Grade 2B).

Some patient subgroups receiving
rivaroxaban experienced increases in
bleeding compared with those receiving
warfarin. These included patients aged
≥ 75 years, and those with prior MI or PAD.
It is important that these factors are
considered when selecting the optimal
therapy for individual patients, but not at the
cost of offsetting important reductions in
other adverse clinical outcomes.

¿Quiénes claramente serían los candidatos
ideales para el uso de los nuevos anticoagulantes?
Pacientes en tratamiento con AVK con un porcentaje de
tiempo en niveles terapéuticos de anticoagulación por debajo
de 50-55%
Pacientes en tratamiento con AVK que presenten ACV o
embolia sistémica, hemorragia mayor o hemorragia
intracraneal
Pacientes con problemas de logística que interfieren con
el seguimiento de laboratorio

¿Quiénes deberían permanecer en tratamiento con AVK?
Pacientes con insuficiencia renal grave (por ejemplo,
depuración de creatinina ≤ de 30 ml / min), o con dispepsia
recurrente (como náuseas, vómitos y diarrea)

De acuerdo a lo señalado en las guías actuales, los
nuevos anticoagulantes ofrecen mejor eficacia,
seguridad y conveniencia comparados con antagonistas
de la vitamina K. Por consiguiente, cuando esté
indicada la terapia anticoagulante, uno de los nuevos
anticoagulantes debería ser considerado en vez de
warfarina para la gran mayoría de los pacientes con
fibrilación auricular no valvular
La evidencia disponible es insuficiente para recomendar
alguno de ellos en particular sobre otro, aunque algunas
características del paciente, cumplimiento y
tolerabilidad a la terapia, así como el costo, pueden ser
importantes consideraciones al momento de elegir el
nuevo anticoagulante
