debería utilizarse una vez completada la fase de inducción, y cuando existe
algún problema para realizar una administración diaria y está garantizada la

1. El tratamiento de la TB pulmonar en pacientes infectados por el VIH que reciben TAR y con evidencia de cumplimiento correcto o bajo tratamiento supervisado durará un total de 6 meses de tto efectivo (2 meses HRZE + 4 meses HR). (Recomendación fuerte,
moderada calidad de evidencia.)
2. En pacientes con cultivos positivos tras 2 meses de tratamiento, el tratamiento se prolongará hasta 9 meses, tras descartar la existencia de resistencia. (Recomendación fuerte, baja calidad de evidencia.)
3. En pacientes con bajo recuento de linfocitos CD4+ en los que no se prevea una respuesta inmunológica adecuada, y en los pacientes en los que no se pueda asegurar una toma adecuada de todas las dosis prescritas, el tratamiento se prolongará hasta 9 meses. (Recomendación débil, baja calidad de evidencia.)

Tto TB-VIH: Inicio de TAR
1.- En pacientes infectados por el VIH con tuberculosis se debe iniciar el TAR durante el tratamiento de la tuberculosis ya que reduce el riesgo de muerte, observándose este beneficio en todos los estratos de linfocitos CD4+. (Recomendación fuerte, alta calidad de evidencia).
2.- En pacientes infectados por el VIH con tuberculosis y cifras delinfocitos CD4+ inferiores a 50 células/mm3 se debe iniciar el TAR a las 2 semanas de empezar el tratamiento de la tuberculosis ya que reduce el riesgo de muerte y desarrollo de sida (Recomendación fuerte,
alta calidad de evidencia).
3.- En pacientes infectados por el VIH con tuberculosis y cifras delinfocitos CD4+ superiores a 50 células/mm3 se debe iniciar el TAR una vez finalizada la fase intensiva del tratamiento de la tuberculosis ya que reduce el riesgo de efectos adversos y síndrome de reconstitución inmune sin comprometer la supervivencia (Recomendación fuerte, alta
calidad de evidencia).

Objective
To assess in ARV-naïve TB/HIV patients with CD4 counts >50 cells/mm3 the
efficacy in terms mortality at 12 months post randomisation of 3 treatment
strategies:
� ARV initiation at week 2 with a standard TB treatment,
� ARV initiation at week 8 with a standard TB treatment,
� ARV initiation at week 8 with high dose rifampicin during the
intensive phase of TB treatment (15mg/Kg) and standard TB
treatment in the continuation phase.

Conclusion
� More aggressive TB treatment using high dose of rifampicin, in addition to ARV treatment, could reduce TB/HIV mortality among co-infected TB/HIV patients with severe immunocompromised state.
� No evidence of an increased risk of hepatotoxicity with higher dosage of rifampicin (15mg/Kg) given daily for 2 months to TB/HIV patients
� More explorations are needed to explain more precisely these results and
PK/PD results will be important to consider
� The results of the RAFA trial provoke an interesting area of further research

Interacciones: Nuevos problemas a la vista
� Raltegravir: Nueva formulación QD (1.200 mg)
• Las concentraciones mínimas descienden con inductores enzimáticos como EFV ¿Rifampicina?
� Tenofovir: Nueva formulación (TAF):
• Contraindicada con Rifampicina:
“Co-administration of rifampicin, a P-gp inducer, may decrease tenofovir alafenamide plasma concentrations, which may result in loss of therapeutic effect and development of resistance.”
Conclusiones
� Se refuerza la evidencia a favor del uso de pautas diarias de tratamiento antituberculoso en pacientes VIH.
� Se mantiene la recomendación de incrementar la duración del tratamiento de la tuberculosis pulmonar sensible sólo en casos seleccionados.
� Se refuerza la necesidad de tratamiento antirretroviral simultáneo y precoz en pacientes con TB muy inmunodeprimidos, mientras que disminuye la fuerza de la recomendación en pacientes con TB y cifras elevadas de CD4.
� Se refuerza el papel de los inhibidores de integrasa (raltegravir y dolutegravir) en el tratamiento antirretroviral simultáneo con Rifampicina, pero es un escenario cambiante y se deben revisar siempre los datos disponibles de los antivirales a utilizar.
