WELCOME TO AMERIGROUP COMMUNITY CARE! Amerigroup Community Care is a community-based health plan offering NJ FamilyCare benefits and more. We also provide long-term services and supports to those who qualify. This provider directory will help you and your Amerigroup doctor work together to make sure you get the right care at the right time.

You can search the directory online at www.myamerigroup.com/NJ. You can print out the results of the online search. Well send you a printed directory if you ask. For more information about the providers listed here, call Amerigroup Member Services at 1-800-600-4441 (TTY 711).

WHAT IS A PROVIDER? A provider is any doctor, dentist, pharmacist, optometrist, specialist, hospital, or other medical group or facility that works together with Amerigroup to provide you with the right care, with access for members with disabilities.

WHO IS YOUR FAMILY DOCTOR? Your family doctor, also called a primary care provider (PCP), is your general health care physician. Your PCP will help you manage the health care needs of you and your family. At Amerigroup, all covered medical services or treatments will be decided by your PCP, except obstetrics and gynecology (OB-GYN), dental, vision and emergency care.

WHAT IS A SPECIALIST? Your PCP can take care of many of your health care needs, but you may also need care from some other kind of doctor. This doctor is called a specialist. Amerigroup works with many different types of specialists. Your PCP can refer you to one if you need special care. Remember, unless you need dental, vision, pregnancy, post-pregnancy or gynecological care, you must have a referral from your PCP to see a specialist. You must also have a referral from your PCP to go to a hospital, except in cases of emergency. See your member handbook for more information.

WHO IS YOUR FAMILY DENTIST? Just as you see your PCP for all your routine medical care, you will see your family dentist, also called a primary care dentist (PCD), for your dental care. By going to your PCD routinely, he or she will get to know you and what your specific dental needs are. If you have special dental needs, your PCD can refer you to a dental specialist.

WHAT IS EMERGENCY CARE? Emergency care is needed if your illness or injury is life-threatening or could seriously hurt your health. In an emergency, call 911 or go to the nearest hospital as soon as possible. Some examples of an emergency are: Very bad bleeding that does not stop Severe pain Chest pains/facial paralysis Very bad burns Passing out

Shakes called convulsions or seizures Trouble breathing Miscarriage Broken bones Throwing up blood Drug overdose or poisoning Rape/sexual assault

WHAT IS URGENT CARE? Urgent care is when you need to see a doctor quickly, but you dont need to go to the emergency room. If you arent sure you need emergency care, call your PCP or the Amerigroup 24-hour Nurse HelpLine at 1-800-600-4441 (TTY 711). They can help you decide. Examples of health issues needing urgent care are: Earaches Muscle sprains/strains Headaches Throwing up Fever over 101 degrees Fahrenheit Sore throat Minor burns or cuts Controllable bleeding

For urgent care that requires you see a dentist but not go to the emergency room, you can have an acute or chronic dental condition such as: Facial swelling due to infection A toothache A tooth that was knocked out or broken

Call your dentist immediately or call Amerigroup at 1-800-600-4441 (TTY 711). They can help you get needed treatment.

HOW DO I GET TRANSPORTATION TO A PROVIDER? Amerigroup covers ground transportation for members in cases of emergency only. Members receive all other transportation services through NJ FamilyCare Fee-for-Service. To find out more about getting a ride to your doctor visits, call LogistiCare at 1-866-527-9933 (TTY 1-866-288-3133).

For routine transportation to your doctor appointments, please call LogistiCare at 1-866-527-9933 (TTY 1-866-288-3133). Transportation does not require approval. However, you must call the LogistiCare number beforehand to have your need reviewed. If you cant get a ride to your appointment, call Member Services at 1-800-600-4441 (TTY 711). If you need emergency care and have no way to get to the hospital, call 911 for an ambulance. If you dont need emergency care but need an ambulance, a mobile intensive care unit or an invalid coach to get to a medical appointment, call LogistiCare at 1-866-527-9933 (TTY 1-866-288-3133).

If you are an Amerigroup member receiving Managed Long Term Services and Supports (MLTSS) benefits, Amerigroup will arrange nonmedical transportation for you. This includes rides to grocery stores, banks and places of worship.

MLTSS transportation services are based on your plan of care and must be approved ahead of time. When possible, you should ask family, neighbors, friends, public transit or community groups for help. MLTSS transportation services are set up by your MLTSS Care Manager. Call us at 1-855-661-1996 (TTY 711) to find out more.

HOW DO I GET CARE IF I HAVE A DISABILITY? All Amerigroup doctors and hospitals will help members with disabilities get care. Members who use wheelchairs, walkers or other aids may need help getting into an office. If you need a ramp or other help, make sure your doctors office knows before you get there. This way, the staff will be ready to help you. If you want help talking to your doctor about your disability needs, call your MLTSS Care Manager at 1-855-661-1996 (TTY 711). A care manager will help you get the care you need.

HOW DO I CHOOSE MY OWN DOCTOR OR DENTIST? Please look at the list of PCPs in this directory. Family doctors include family practitioners, general practitioners, pediatricians (for children) and internal medicine doctors. You may already know a family doctor, or a friend may be able to recommend one. You should choose a PCP who is easy for you to get to. Each person in your family can choose a different PCP.
