Welcome

This directory will help you find doctors and hospitals in your area who work with UnitedHealthcare

Community Plan in Washington. We work with primary care physicians (PCPs), hospitals, specialists

and other providers for your health care needs. We are a health plan partner with the state of

Washington. Your health care will be provided by doctors who are in our provider network. You

can find a current list of providers on our website at myuhc.com/CommunityPlan. If you need help

finding a doctor or using this directory, call Member Services at the number below. We'll be happy

to help you.

How to Use This Directory

Choosing a Primary Care Physician (PCP)

There are several types of PCPs in this directory. Each covered family member may choose a

different PCP in UnitedHealthcare Community Plan. Except in an emergency, your PCP and staff

will give or manage all your health care. This includes referrals to specialists. PCPs may be family

or general practitioners, internists, pediatricians, or other providers approved by UnitedHealthcare

Community Plan.

Changing Your PCP

If you want to change your PCP, call Member Services. They will help you pick a new PCP. You may

change your PCP during the year. You may also contact a provider to find out if the provider contracts

with UnitedHealthcare Community Plan. When you call, be sure to mention UnitedHealthcare

Community Plan. Ask if the provider is taking new patients.

Seeing Another Doctor or Specialist

Your PCP might want you to go to a specialist. A specialist is a doctor who treats a special problem,

like a dermatologist or a heart doctor. Your doctor will help you find the right specialist. If you have

a complex illness, you may need to see the specialist many times. You may see your specialist as

often as needed. If you have complex care needs, a specialist can be your PCP. If you need this, call

Member Services. We will help you find a specialist who can meet your needs.

Women's Health Care Services

If you are female, you may want to see a provider in our network who offers women's health care.

You can get these services from an in-network women's health care provider without a PCP referral.

These women's health services are covered without a PCP referral or health plan preauthorization:

Maternity care, including prenatal, delivery, and postnatal care.

Routine gynecological exams.

Examination and treatment of disorders of the female reproductive system.

Other health problems treated during a woman's health care visit, if the treatment is within the

provider's scope of practice and the service is not excluded.

If you need to see a women's health provider who is not in our network, you must get a referral from

your PCP.


How to Use This Directory (continued)

Out-of-Network Providers

You or your PCP might decide that you need to see a doctor or provider that is not in our network.

Your PCP will need to call us for an okay for these services before they will be covered. This is called a

Prior Authorization.

Pharmacy

For locations of pharmacies, call Member Services at 1-877-542-8997, TTY 711.

What if I need medical care when I am out of town?

We will pay for routine care out-of-area only if:

You call your PCP first and he or she says that it is important that you get care before you return home.

Your PCP must then call us to get approval.

If you do not speak to your PCP before you get routine care when you are away from home, you

may have to pay for care yourself. If you cannot reach your PCP, call Member Services.

Any doctor you see must agree to accept Washington Medicaid payment as payment in full.

This means if you or your family members are on vacation and need routine care, we will pay only if

you get our approval first. If you need emergency care when you are out of town, go immediately to

the nearest Emergency Room (ER) or urgent care facility. You do not need a referral from your PCP.

Call your PCP as soon as you can after getting ER or urgent care services.

No Medical Coverage Outside of the United States

Any health care services you get while out of the country will not be covered by UnitedHealthcare

Community Plan. Medicaid cannot pay for any medical services you get outside of the United States.

Language Help

We can give you member materials in a language or format that is easier for you to understand. We

have interpreters if your doctor does not speak your language. This is free when you talk to us or your

doctors. If you do not speak English and need help, call Member Services at 1-877-542-8997, TTY 711.

Emergency Care

In an emergency, you do not need a prior authorization. Call 911 or go to the nearest emergency

room. You can get emergency care 24 hours a day, 7 days a week.

To find a provider, go to the table of contents and find the listing for your county. The providers are

listed by their specialty. When you find the specialty you want, go to the page listed. Then you can pick

the provider who meets your needs. Members may get care from a resident physician or a certified

registered nurse practitioner under the supervision of a PCP.

UnitedHealthcare verifies this information when these providers join our network and every three

years (or sooner) when the provider is re-credentialed.

Board Certification Board certification

shows a physician's expert knowledge in a

medical specialty. Certification involves a

process of testing and evaluation by specialists

in the specific area of medicine. A board-

certified specialist participates in continuing

education to learn about the latest advances

in medical science and technology in his/her

specialty. You can check the provider's

board certifications on the American Board

of Medical Specialists (ABMS) website.

Cmo se debe emplear este directorio

Hay varios tipos de PCP en este directorio. Cada uno de los miembros de la familia puede escoger

Servicios para los Cuidados de Salud para la Mujer

Si usted como mujer quiere ser examinada por un proveedor de nuestra red que ofrezca cuidados de

salud para mujeres, usted puede obtener esos servicios sin necesidad de ser referida a ellos por un

PCP.

Esos servicios de salud para mujeres que se cubren sin necesidad de una referencia de un PCP o sin

necesidad de una preautorizacion del plan de salud son:

Cuidados de maternidad, incluyendo los prenatales, parto y cuidados de postparto.

Exmenes de rutina ginecolgicos.

Examen y tratamientos de problemas del sistema reproductor femenino.

Otros problemas de salud que se han observado durante las consultas para los cuidados de

ese servicio no est excluido.

Proveedores fuera de nuestra Red

Usted o su PCP pueden decidir que usted necesita consultar con un doctor o un proveedor de salud

que no se encuentra dentro de nuestra red. Su proveedor necesitar llamarnos para que nosotros

autoricemos estos servicios antes de poder cubrirlos. A esto se le llama Autorizacin Previa.

Qu pasa si necesito atencin mdica cuando estoy fuera de la ciudad?

Nosotros cubriremos el costo de cuidados de salud de rutina en sitios fuera de su rea solamente si:

Usted llama primero a su PCP y l o ella dice que es importante que usted reciba tratamiento

antes de que retorne a su casa.

Su PCP entonces debe llamarnos para obtener aprobacin.

Si usted no se comunica con su PCP antes de obtener cuidados de salud de rutina cuando

se encuentra fuera de su rea, es posible que usted tenga que cubrir el costo de estos servicios.

Si usted no puede comunicarse con su PCP, llame a Servicios para Miembros.

Cualquier doctor con el que consulte tiene que estar de acuerdo con aceptar el pago de

Washington Medicaid como pago total.

Eso significa que si usted o cualquier miembro de su familia se encuentran de vacaciones y necesitan

de cuidados rutinarios de salud, nosotros pagaremos por ellos solamente si usted obtiene una
