CONCLUSIONES 1. En ptes con SCA, seleccionados, estabilizados y con un gran trombo intracoronario,
diferir la PCI y la implantación del stent después de un intensivo tratamiento antitrombótico (IIb/IIIa+enoxaparina) parece ser segura.
2. Diferir la PCI puede estar asociada con una reducción en la carga trombótica y de complicaciones angiográficas (no/slow reflow, embolización distal y complicaciones trombóticas intraprocedimiento) así como de un mayor porcentaje de miocardio salvado , sin un incremento de las complicaciones hemorrágicas.
3. En lo posible implantar un stent bastante más largo a lo usual, en forma directa, sin post-dilatación y considerar el Stent Mguard.
4. La tromboaspiración manual es inefectivo en aspirar grandes trombos en cerca de un 1/3 de los casos y debido a su alto perfil, hay riesgo de embolización distal al cruzar la lesión. Además, suele dejar trombo residual que puede ser el origen de embolización al implantar el stent.
5. Los dispositivos de protección distal parecen reducir la incidencia de complicaciones tromboembolicas, especialmente útiles en la CD y Puentes Safenos, pero los estudios han fallado en demostrar algún beneficio.
