A LIVING DONOR STORY, 10 YEARS AFTER BY DAN MARTINSEN

Thisisastoryoffamilyandfaithandtwowarrior-angels.It’sahappystoryandasadstory.It’sastorywithanendyettobewritten.Tenyearsago,amedicalteamatNewYorkColumbiaPresbyterianHospitalconductedits irst-everlivingdonorlivertransplantsurgeryonacancerpatientinanattempttosavehislife.ThepatientwasKevin,myyoungerbrother.Iwasthelivingdonor.

ShortlyaftersixonthemorningofMay24,2007,weweregatheredintheoper-ationprepareaofColumbia’sMilsteinHospital:MybrotherandI,Kelly(hiswife),Doreen(mywife),andourpar-ents,DanandBettyMartinsen.

KevinandI,standingwithintravenouspolesatourside,thoughtwerockedourmatchingblue-greenhospitalgownsandtan,no-slipsocks.Ourwivesmadejokesandsmalltalktokeepourmomdistracted.Asidefromus,itwasanemptyandcoldroom.Kevinwastakenintotheoperatingroom irst.Theywouldperformanex-ploratoryproceduretocheckforcancercells.Ifhewerecancer-free,Iwouldfollow,andourlivingdonortransplantoperationwouldproceed.However,ifanycancerweredetected,theywouldbeforcedtostopandpursue“otheroptions.”Ninetyminuteslater,hissur-geonappeared,anddroppedhermasktorevealasmile.Nocancercells.InIwent.

Earlierthatyear,Kevinhadbeendiag-nosedwithPSC(primarysclerosingcholangitis),arareliverdiseasethatslowlydamagesthebileductsandcanleadtobileductcancer(cholangiocarcinoma),aparticularlyvirulentdiseasewithlowsurvivalrates.

KevinandIweredrivinghomefromaNYGiantsfootballgamewhenhetoldme.Thedisease,hesaid,wasinitsearlystages;hemightneedatransplant…manyyearsfromnow.Butinthatmo-ment,avoiceinmyhead,theonethatusuallytellsmeIforgotmykeysorsomethingofroughlyequalimportance,

said,“Hewillneedatrans-plant,andyoushouldgetready.”

Kevinistheyoungestinourfamily—10yearsyoungerthanI;eightyearsyoungerthanourbrother,Bob;andsixyearsyoungerthanoursister,Marianne.Youcandothemath; irst,therewerethreekids,evenlyspacedtwoyearsapart,andthen—sixyearslater—Kevin.WeCatholicscallthata“pleasantsurprise.”

Hewasthe“baby,”and,assuch,receivedababy’sat-tention.Irememberthedayourmombroughthimhomefromthehospital,shinyandnew,andscreaminglikehell.Mybroth-erandsisterandIwereasexcitedasifitwereChristmasmorning.Mariannecarriedhimaroundonherhipcallinghimher“KevyCakes.”Whenhewasolder,Iwouldoccasionallytakehimwithmetohighschoolvarsitybaseballgames.Ihadanulteriormotive;at14,Iwasshyaroundgirls,butI’dlearnedthatnothingshortofapuppycouldgetgirlstotalktoyoufasterthanacute4-year-oldproppedonyourshoulders.Inhisteenyears,Kevingotattentionfor,letussay,“other”reasons.Onmorethanoneoccasion,ourbrother,Bob,andourdadwoulddrivearoundourhometownofCentereach,LongIsland,lookingforhimwhenhehadmissedcurfew,callinghisnameoutfromthecar.

ButthenhemetKelly.Inher,hefoundpurpose.Inher,hefoundtheloveofhislife.TheymarriedandsettledinLongBeach,LI,readytotakeontheworld.Andtheydid.

Wewereablessedfamily;we’dneverfacedagenuinecrisis,anddidn’tfeelwewerefacingonewithKevin’sinitialdiagnosis.ButacoupleofmonthsafterthatridehomefromtheGiantsgame,wereceivednewsthatmoreorlesspar-

alyzedus.

Duringaroutinetest,Kevinhadtestedpositiveforbileductcancer.

WeGoogledit,andbegantorealizehowseriousthiswas.KevinandKellywenttoseeDr.RobertBrownandDr.JeanEmondatTheCenterforLiverDiseaseandTransplantationatNewYorkCo-lumbiaPresbyterianHospital.Fortu-nately,thecancerhadbeendetectedearly,whichprovidedtheopportunityforwhatwasthenabrandnewap-proach:First,treatandkillthecancercellswithradiationandchemotherapy,andthenreplacetheliverandcommonbileductviatransplantassoonaspossi-ble.However,therewasanotherhurdle.Kevin'sliverMELD(ModelforEnd-StateLiverDisease)scorewouldhavehadhimwaitingalongtimeforacadaverliver.IntheUnitedStates,thedemandforvitaltransplantorganscontinuestosurpasstheavailability,andtheMELDsystemprioritizesorgansbasedonmor-talityrisk.InKevin’scase,hisliverwasstillfunctioningwell,but,ironically,bythetimehewouldhavequali iedforacadavertransplant,hiscancermostlike-

Iawoketothenewsthattheoperationhadbeensuccessful.Thedoctorshadremoved70%ofmyliver(itistheonlyorganthatre-generates)andputitinmybrother,aswellasreconstructinghiscommonbileduct.Theytookourgallbladders,too,forgoodmeasure(Whoreallyneedsagallbladder?).Iadmitthenextfewdaysinthehospital,especiallyaftertheinitialpainmedica-tionsworeoff,weren’texactlyaMardisGrasparadeforeitherofus,orourwivesorparents.

Duringthose irstpost-opdays,it’smainlynurseswhogetyouthrough,andIbelieveeverypatienthasanursethatis“TheOne.”Forme,itwasBer-niceGonzalez.Iwouldwakeuphopingtoseehernamewrittenontheassign-mentchalkboardinfrontofmybed.Bernicewassmart,strong(hell,sheliftedmeupandIamnotasmallfel-low),empathetic,positiveandkind.Iknewherforallof ivedays,andIamsureshe’shelpedthousandsofpeopleduringhercareer,butshemademefeelasifIweretheonlypatientunderhercare,andalthoughshemayforgetme,Ishallneverforgether.

Duringourtimeinrecovery,DoreenandKelly,ourwarriorangels,stoodguard.Sleepinginashareddormroomatthehospital,theywereourtirelessadvocateswiththedoctorsandotherhospitalpersonnel,plustheysuccess-fullykeptatbaythemanywell-wishers(sometimesevenourparents)asKevinandIandwaitedforourgastrosystemstorestart.Plainlyput,inordertorecov-er,weneededtomusterupsomegas.Yes,therewere,inthehospitalchapel,seriousprayersforfarts.Whentheyinallycame,theentire loorexploded(sic)incelebration.

KevinandIwerekeptinseparaterooms,butoccasionallywemettodosome“laps”aroundtheward.Wewereweak,scruffyandskinny,butonceagainwerockedthosehospitalgowns,feetysocksandIVs.Orsowelikedtothink.

Meanwhile,DoreenandKellywouldget

lywouldhavespreadbeyondthepointofthetransplantbeingsuccessful.

DoctorsBrownandEmondbroughtupthelivingdonoroption.

Necessitybeingthemotherofinven-tion,thelivingdonoroptionwasemergingasalife-savinganswertothelackofavailabilityofcadaverorgans,butitwasarelativelynewprocedure,andhadneverbeenattemptedonacancerpatient.

AlthoughKevinunderwentradiationandchemotreatments,herefusedtoallowanyoneinourfamilytovolunteertobealivingdonor.Hepursuedotheroptions,includingwhatwouldhavebeentransplantwitha“compromised”cadaverliver.Meanwhile,precioustimewaspassing.Thiswaswhenmywifekickedourfamilyintheass.SaidDoreen:“YouguyshavetosnapoutofitandstopwaitingforKevintomakeadecision.Wehavetopersuadehimtoletoneofusbethedonor.Thisisreal,andweneedtomovefast!”Thatmo-mentgalvanizedthefamily.Wecollec-tivelytoldKevinhemustacceptoneofusasapotentiallivingdonor.Iwastested irst.

Itwasamatch.

Ididthiswiththe100%encourage-mentofDoreen,nevermindthatwehadthreechildren,jobs,ahouse,andourownfearsofwhatmighthappen.

Amonthafterpre-tests,KevinandIcheckedintothehospitaltoundergoColumbia’s irstlivingdonorlivertrans-plantsurgeryasatreatmentagainstbileductcancer.

KevinandIwereinthehandsofDoc-torsEmond,BenjaminSamstein,SarahBellemare,andtheirteams.Lookingback,Ibelievethetoughestpainoftransplantsurgerymaybetheoneyourfamilyendures.KevinandIwereunderanesthesia.Wewere“elsewhere.”Butoutsidetheoperatingroom,thereweretwowives,andtwoparentswhohadjustnowwatchedtwooftheirsonsgointoaserioussurgery.Theywereall

upat ivetogetcoffeearoundthecor-nerfromthehospital.Theybecamemini-celebritieswiththeguysatthelocal irehouse,whowouldseethesetwopossiblycrazyblondeswalkingup168thstreetintheirpajamas,inthedark.

Thesewomenspenttheirdayssupport-ingus,whilealsoworkingtheirjobsanddrivingtoandfromhometotakecareofthekids.

Aftersixdays,Ileftthehospitalwithadrainagetubejuttingoutfrommyab-domenduetoabileleakage,whichisacommoncomplicationfromthesur-gery.Thisremainedinmefor10days.Recoverytookfourmonths;I(temporarily)lost25pounds;andforthe irstfewweeks,lookedlikeTimConwayfromTheCarolBurnettShow,doinghischaracter“TheOldMan,”shuf lingaroundthehouseandtheneighborhoodinmyredbathrobewhilemyliverregeneratedandmystrengthreturned.Byautumn,Iwasbacktowork,andrunningfourmilesaday.

KevinandKellyhadalongerroadaheadofthem.Thetransplanthadworked,buthehadtoenduremultiplefollow-upsurgeriestoaddressabileleakagethatwouldnotstop.Bileispoi-son.Ithadgottentothepointwhereanewtransplantwasbeingconsidered.Butthen,anothersurgeon,Dr.TomoakiKato(whorecentlyhadcomefromMi-amitojointheColumbialivertrans-plantteam)andDr.JonathanSussmanintheInterventionalRadiologyDepart-mentatColumbiafoundawaytosuc-cessfully ixtheissuewithoutanewsurgery.

Idon’tevenletmyselfthinkaboutwherewe’dbetodaywithoutthebril-lianceofoursurgicalteamsandtheempathyofourpost-opnurses.WhatIdofocusonisthestrengthandfaithexhibitedbyeveryoneinmyfamily,particularlyDoreenandKelly,andmyparents.KellyandDoreenpossessstrengthforgedfromchallengestheypreviouslyfacedintheirownlives.Theyusedthoseexperiencestohelp
