Factores de riesgo: cirugía previa, índice de Charlson, IMC, duración del procedimiento
La pauta de profilaxis ampliada fue un factor protector (P = 0.036)
CONCLUSIONES
1. Aunque de momento no existe una fuerte evidencia, la profilaxis quirúrgica dirigida
en pacientes colonizados por BGN multirresistentes debe ser considerada en
pacientes de alto riesgo y cirugías con alta tasa de complicaciones infecciosas
2. Los regímenes elegidos para realizar estas profilaxis, deberían ser “sostenibles
ecológicamente” y evitar en la medida de lo posible el uso de carbapenémicos

ALGUNAS CUESTIONES EN EPC….
No existen ensayos clínicos y los estudios que hay son heterogéneos
La mayoría de los estudios están hechos con KPC
No existe mucha información con ceftazidima-avibactam
Los datos sugieren que en pacientes graves y fuentes de infección “no urinarias”, podría estar indicada la biterapia
CARBAPENÉMICOS
Con MIC < 4 mg/L y administrados en perfusión continua, se podrían alcanzar objetivos farmacodinámicos
Sin embargo, no existen datos suficientes en la clínica para recomendarlos en monoterapia aunque sean sensibles, fuera de la infección de orina
En infecciones causadas por KPC, es adecuado añadir meropenem a otros fármacos activos, si la MIC es < 8 mg/L. No existen datos suficientes en OXA-48 y MBL
Ojo!!! Su uso puede favorecer la aparición de altos niveles de resistencia (alteraciones de permeabilidad, expresión de carbapenemasas….

CONCLUSIONES
El tratamiento antibiótico empírico de pacientes colonizados, debería incluir al menos un agente activo contra los microorganismos aislados en los frotis de vigilancia
Los tratamiento elegidos deberían considerar el impacto ecológico de su uso
El estado del paciente y el foco de la infección deben tenerse muy en cuenta a la hora de diseñar el tratamiento empírico
Tener en cuenta las características del antibiótico elegido: metabolismo, farmacocinética…..
