Abstract

Background: Accurate diagnosis is a fundamental aspect of appropriate healthcare. A diagnostic test is any kind of medical test performed to aid in the diagnosis or detection of disease. Different tests are done to diagnose diseases, and preferably sub-classify it regarding, for example, severity and treatability, and to confirm that a person is free from disease. However, clinicians need guidance when implementing diagnostic tests given the number of tests available and resource constrains in healthcare. Practitioners of health often feel compelled to implement recommendations in guidelines, including recommendations about the use of diagnostic tests. The main purpose of the study was to assess the healthcare privileges and access for urban and rural people of Bangladesh.

Materials and Methods: Several diagnostic centers of whole Bangladesh visited to accumulate data. Urban people were interviewed person to person in random availability basis based on a previously prepared questioner form developed by experts.

Discussion: The accomplished data were analyzed statistically and compared between Dhaka and other area of Bangladesh. Local people of urban area have a concept that government should be more conscious about the health sector and all types of tests should be available government hospitals and the number of government hospitals should be increased.

Abstract

Results: It was obtained that the rural people think Diabetes and Cardiovascular disease are more complicated and dangerous to them. Almost in every case they use to go to the specialist doctor and Government health officers. It was found that the cost of Diagnostic tests are nearly 104% higher in Dhaka city than that of rural area. About 31% people who rely on 100% diagnostic tests or tools, in 44% people rely on 90%,15% people rely on 50%, and 5% people rely partially on diagnostic test results. And there were about 5% people who do not rely any diagnostic tests results. It was also found that almost 24% rural people go to diagnostic center for blood test. And X-ray 20% most of the people think ECG are serious. From this analysis I also discovered that nearly 70% of rural people have enough faith on their diagnostic tests. In primary healthcare problem 39% rural people generally go to Local medical practitioner,4% go for specialist doctors and 27% people would like to go for MBBS doctor. But the most surprising finding was 57% specialist doctors suggest them for repeating or redo the test when they came from urban area to Dhaka for better treatment.  

Conclusion: The main objective of this project work was to find out the diagnostic cost between the rural and urban diagnostic centers, relatively of tests equipment facility. Here our findings will help us to study such matter in coming future. It should be mentioned that it is necessary to study such think to assist people of urban areas as well as Dhaka city.

different diagnostic tests, cost variation in different diagnostic centers and compare the costs of different diagnostic tests of rural areas with others diagnostic centers of Dhaka, Bangladesh.  

The study also analyze the concept of general people about diseases, their idea about when a patient goes to the physician, their thought serious diseases, where they go in case of diseases-government, private or others clinics, why peoples are less interested to go to government clinics, which type of people go to the government clinics, which type of health problem people think serious or complicated, what types of diagnostic tools or tests are used in serious or complicated health problems, the reliability about diagnostic tools.

By asking the people that How can we minimize this diagnostic costs? Then we got answer : 40% people said Government can take steps against it, 35% have no comment and 25% said to take some tactic.

When we asked that, do Doctors in Dhaka tell to redo the tests that are done in locality? Then we got answer that : 57% said yes, sometimes doctor do that.38% said thay have no experience and 5% depends on their physician.

Conclusion The study reveals that there are significant

differences among the costs of diagnostic tests in remote areas with the costs of diagnostic tests in Dhaka. Besides, some centers were found to provide false report of diagnostic tests. Though it is mandatory, most of the diagnostic centers were found to have no list of the costs of diagnostic tests. The diagnostic centers charge against the test as their own wishes. There have no any accountability. Some diagnostic centers appoint below qualified technologist. There are also many unregistered clinics and clinics having insufficient instruments. Some government hospitals and clinics have some sophisticated instruments but lacks traind operators to run the instruments. Besides, some diagnostic centers do not maintain standard environment and health and hygiene. Authority is not so serious about that. As a result people never get the appropriate services by those types of clinics or diagnostic centers.

Conclusion Precise detection of disease is the prerequisite of proper

treatment. So, diagnosis have to be accurate. There may many reasons behind the variations of costs of diagnostic tests in remote areas with the costs of diagnostic tests in Dhaka. Firstly variation of cost of diagnostic tests may be due to use of sophisticated instrument and state of the art technology; secondly, costly maintenance; thirdly, appointment of more qualified technologist in Dhaka, fourthly high operating cost in Dhaka city from local. Fifthly highly business purpose and not update the upcoming cost list of diagnostic tests and also use the poor machineries. On the other hands, people are not so aware about the problem and they never ever know where to complain to the authority. Furthermore, conscious people never take necessary steps to against those problems, So, problem are growing day by day.
