IntroducciónLeucemia Mieloide Aguda
La leucemia mieloide aguda (LMA) es una enfermedad
neoplásica en la que se produce una proliferación anómala
de células inmaduras (blastos) de estirpe mieloide.
Las células blásticas proceden de un único progenitor
hematopoyético defectuoso que presenta un bloqueo en el
proceso normal de maduración así como un aumento de su
capacidad de proliferación.

Hypothesis
Alterations of different components of the Wnt pathway have
been found in some hematological malignances providing
evidence for the involvement of the Wnt pathway in
leukemogenesis.
We hypothesize that the functional loss of Wnt antagonists
by promoter hypermethylation is a possible mechanism
contributing to activation of the Wnt pathway in AML and that
this loss may be involved in the pathogenesis and prognosis
of this disease.

Objectives
1. To study the methylation status of the Wnt antagonists: sFRP1,
sFRP2, sFRP4, sFRP5, DKK1 and DKK3 in cell lines of
myeloid lineage as well as in a large series of 184 patients
diagnosed with de novo non-M3 AML.
3. To demonstrate the association between aberrant methylation
of Wnt antagonists and activation of the Wnt pathway in AML,
analyzing the expression of Wnt downstream genes (cyclin D1,
TCF and LEF) and the location of β-catenin using cell cultures
treated with DAC.
2. To evaluate the impact of aberrant methylation of Wnt
antagonists in gene expression using the hypomethylating
agent decitabine (DAC).

4. To analyse the association of any individual gene and the
methylation profile (group A: patients with 0-1 methylated
genes; group B: patients with 2-6 methylated genes) with the
clinical and biological characteristics of the AML patients.
Objectives
5. To evaluate the prognostic impact of the aberrant methylation
of Wnt antagonists on the outcome of the AML patients.

Conclusions
1. Aberrant methylation of the Wnt antagonists is a frequent event
in cell lines of myeloid lineage and in patients diagnosed with
AML. Indeed, we have found concurrent methylation of two or
more genes in 43% of the AML patients.
2. Aberrant promoter methylation results in gene silencing of Wnt
antagonists.

3. Treatment with DAC of the AML cell lines permit the re-
expression of the Wnt antagonists and therefore, induce a
reduction of β-catenin in the nuclear localization and
downregulate the expression of the Wnt downstream genes. In
concordance, the transcription levels of cyclin D1, TCF and LEF
are significantly higher in patients showing more that two Wnt
antagonist methylated genes. This is a strong evidence that
epigenetic regulation contributes at least in part to the activation
of the Wnt pathway in AML.

Conclusions
4. Aberrant methylation of any individual gene and the methylation
profile are not associated with clinical and biological
characteristics of the AML patients in our series. The exception
is for aberrant methylation of sFRP5, which is detected more
frequently in patients with an adverse prognosis. Therefore,
methylation of sFRP5 might provide valuable information about
poor outcome in AML.
5. Methylation of two or more Wnt antagonistic genes is a new
adverse prognostic factor in the subgroup of patients ≤ 60 years
with intermediate-risk cytogenetics.
