Die „Lapidus“-Arthrodese mit der „TOTALFIX“-Platte
Die Lapidus-OP Technik stellt eine Korrektur des Intermetatarsalwinkels im TMT I Gelenk dar. DieFixation wurde in der Originalarbeit von Lapidus (1934) nur mit einem Gips ausgeführt. Das hat zuzahlreichen Pseudoarthrosen und Rezidiven geführt. Deshalb wurde diese Methode recht baldwieder verlassen.
Die Schraubenosteosynthese von dorsal war die Neuauflage in den 90er Jahren. Die Schrauben, oderSchraube und Draht, oder Schraube und Staple wurden von dorsal eingebracht. Das führte zu einerguten Korrektur und guten Durchbauungsraten, die Patienten mussten aber 8 Wochen entlasten.Damit war die Indikation eingeschränkt.
Orthner/Wels entwickelte die winkelstabile Stufenplatte von dorsal. Damit war die Plantarverschiebunggelöst, nicht aber die Belastbarkeit. Die Implantation einer winkelstabilen „Totalfix-Platte“ von medio-plantar bringt den entscheidenden Vorteil. Die Verspannung der Arthrodese erfolgt biomechanischkorrekt im Sinne der Zuggurtung, d.h. wenn der Patient nach der Operation teilbelastet, komprimierter die Arthrodese und verbessert die Durchbauung. Die Teilbelastung im Vorfußentlastungsschuherleichtert die Nachbehandlung wesentlich. Damit kann die Indikation ebenfalls erheblich ausgedehntwerden.
The “Lapidus” arthrodesis with the “TOTALFIX“ plate
The Lapidus arthrodesis technique corrects the intermetatarsal angle of the TMT-I joint. The originaltechnique used by Lapidus (1934) only fixated the joint with a plaster cast; this resulted in numerouscases of pseudoarthrosis and relapses, so that surgical procedures soon departed from thistechnique.
The screw osteosynthesis from the dorsal side was the new version in the '90s. The screws, or screwand wire, or screw and staple, were inserted from the dorsal side. This led to a good correction andimproved the bone building, but the patients had to refrain from putting pressure on the joint for8 weeks. This restricted the indication.
Orthner/Wels developed the angle stable step plate for the dorsal approach. This solved the problemof plantar displacement, but not the loading ability. The implantation of the angle stable“Totalfix plate" from the medio-plantar side makes a crucial difference. The arthrodesis is correctlytensioned by biomechanical means along the lines of a tension band, i.e. when the patient appliespartial load after the operation, he compresses the arthrodesis and improves the bone buildingprocess. Partial load in the forefoot off-loading shoe considerably simplifies post-surgery treatment.This results in a considerable expansion of the indication.
La artrodesis de “Lapidus” con la placa “TOTALFIX“
La técnica de “Lapidus” significa la corrección del angulo intermetatarsal dentro de la articulación TMT I.Al llevar a cabo el trabajo original de Lapidus (1934) la fijación fue realizada solamente conescayola lo que resultaba en numerosos seudoartrosis y recidivas. Debido a eso el métodomencionado fue suprimido.
La fijación dorsal con sistemas de osteosíntesis fue aplicado nuevamente en los años 90. Los tornillos,o tornillo y aguja, o tornillo y grapa fueron implantados desde dorsal. Eso llevó a una correccióndeseada y una buena regeneración ósea, pero los pacientes tuvieron que descargar durante 8semanas. Por eso la indicación resultó limitada.
Orther/Wels desarrolló la placa de bloqueo con escalón desde dorsal. Con ello la plantarizacióninmediata era solucionada, al contrario de la carga. La implantación de la placa “Totalfix” de bloquedesde medio plantar significa la solución. La fijación de la artrodesis sucede biomecánicamentecorrecto en mente del effecto de “tension band”, o sea que si el paciente carga parcialmentedespués de la operación, comprime la artrodesis y mejora la regeneración ósea. La carga parcial enla bota post-quirúrgica facilita el tratamiento post-operativo considerablemente. Esto tambiénpermite una ampliación bastante de las indicaciones.
