A ventricular catheter connected to an external strain gauge transducer is themost accurate and cost effective method of monitoring ICP.
A ventricular catheter offers the possibility to obtain CSF and it is used as treatment in lower ICP therapies.
Ventricular pressure measurement is the reference standard for ICP monitoring.
ICP measurement by parenchymal micro strain gauge pressure transductionis similar to ventricular ICP.
Fluid coupled epidural devices or subarachnoid bolts and pneumatic epidural devices are less accurate than ventricular ICP monitors

¿Qué miramos en el microdializado en el laboratorio?1.Perfil temporal de los mediadores de respuesta neuroinflamatoria (expresión de interleucinas)
2.Perfil molecular del edema cerebral postraumático (iónico y macromolecular)
3.Marcadores de integridad de la BHE. (expresión de MMP’s)
Periodo de monitorización del paciente (máximo 120 horas consecutivas) se determinan hora a hora las concentraciones de: glucosa, lactato, piruvato, glicerol y glutamato, así como el índice Lactato/Piruvato

SANGRE Y/O PLASMAEste tipo de muestras se utilizan como referencia o bien para estudios a nivel sistémico.Actualmente estamos estudiando el perfil temporal de la respuesta inflamatoriasistémica tras un TCE grave.

SURGICAL MANAGEMENT OF TRAUMATIC PARENCHYMAL LESIONS
• Patients with parenchymal mass lesions and signs of progressive neurologicaldeterioration referable to the lesion, medically refractory intracranial hypertension, or signs of mass effect on computed tomographic (CT) scan should be treated operatively.
• Patients with Glasgow Coma Scale (GCS) scores of 6 to 8 with frontal or temporal contusions greater than 20 cm3 in volume with midline shift of at least 5 mm and/or cisternal compression on CT scan, and patients with any lesion greater than 50 cm3 in volume should be treated operatively.
