Antecedentes personales
• HTA diagnosticada hace 6 años, en tratamiento con valsartán 160 mg/día, con aparente buen cumplimiento. Refiere tener unas cifras tensionales habituales de AMPA por la mañana de 135/85 mmHg. Alguna vez se ha medido la PA por la tarde, con cifras en torno a 150/85 mmHg.
• Obesidad abdominal.
• Vida sedentaria.
• Fumadora hasta hace 1 año de 10 cig/día durante 20 años.
• Le han comentado que tiene glucosa “alta”.
Caso clínico
Enfermedad actual
Refiere disnea a moderados esfuerzos, que no ha
progresado en los últimos meses. Leves edemas maleolares, sobre todo en verano.
Además del tratamiento habitual toma simvastatina 20 mg/día.
Exploración física
• PA (media de 3 tomas): 148/93 mmHg
• Frecuencia cardiaca 70 lpm
• Peso: 82 kg, talla: 162 cm; IMC: 31,24 kg/m2; perímetro de la cintura: 100 cm.

• Hemograma normal.
• Bioquímica:
Glucemia en ayunas 134 mg/dl. Hba1c: 6.6%.

• No existen evidencias que apoyen la reducción de PA a valores <130/80 mmHg para prevenir eventos cardiovasculares.
• Todos los grupos de antihipertensivos son eficaces para reducir el riesgo.
• La combinación es necesaria para reducir la PA. Un fármaco bloqueante del SRA siempre debe estar incluido debido a las evidencias de su efecto protector sobre el inicio y la progresión de la nefropatía.

Estrategias para consecución del objetivo
• La mayoría de los pacientes hipertensos necesitan para controlar de forma eficaz la PA la combinación de al menos dos fármacos.
• El inicio en combinación tiene ventajas en los pacientes de alto riesgo donde el control de la PA debe ser precoz.
• Deberían preferirse las combinaciones a dosis fijas para simplificar el tratamiento y facilitar el cumplimiento.

• Diabetes mellitus (en tratamiento con insulina nocturna y una dosis de metformina al mediodía.
• Obesidad abdominal.
• Vida sedentaria.
• Sin hábitos tóxicos conocidos.
• Prótesis de cadera derecha y coxartrosis izquierda (no se quiere poner la otra prótesis).
• Dolor precordial atípico hace unos meses con ECG y troponina normal. Está pendiente de valoración por Cardiología.

Estrategias para consecución del objetivo
• En no menos del 15-20% de los pacientes hipertensos, no se
alcanza el control de la PA con una combinación de dos
fármacos. Cuando se necesitan tres fármacos, la
combinación más racional sería un bloqueador del sistema
renina-angiotensina, un antagonista del calcio y un
diurético a dosis eficaz.
