• Background: The STREAM trial compared the strategy of primary PCI vs. fibrinolytic therapy
with bolus tenecteplase before transport to a PCI-capable hospital in 1.892 patients with
STEMI who presented within 3 hours after symptom onset and who were unable to undergo
primary PCI within 1 hour
30 day results (Armstrong et al NEJM 2013):
- Prehospital fibrinolysis with timely angiography had a similar composite endpoint as primary PCI
- A small increased risk of intracranial bleeding
- A non-significant 1.5% lower incidence of cardiogenic shock and acute heart failure
• Aim: To determine the long term effect (1 year follow-up) of a pharmaco-invasive strategy
(fibrinolysis coupled with timely coronary angiography) versus primary PCI
• Design: Open-label, prospective, randomized, parallel-group, multicenter trial, N=1168,
observation period 1 year
• Primary endpoint: composite of all cause death, or shock, or CHF, or reinfarction; objective
presented: 1 year mortality
