Abordaje de la hepatitis C en 2016
Los nuevos tratamientos con AADs son capaces de erradicar la infección por VHC en más del 95% de los casos en pautas de 8-24 semanas, con un excelente perfil de seguridad y sin límite de edad.
Una parte importante de los pacientes con hepatitis C tienen edad y/o enfermedad hepática avanzadas, presentan comorbilidades y reciben tratamientos concomitantes que deben tenerse en cuenta a la hora de prescribir el tratamiento con AADs.
La hepatitis C es frecuente en los pacientes con insuficiencia renal crónica grave y estos pacientes constituyen un subgrupo de difícil manejo terapéutico.

Mensajes en pacientes con enf. hepática avanzada
• El tratamiento con AADs es globalmente eficaz, seguro y bien
tolerado en pacientes cirróticos.
• Los pacientes descompensados tienen peor respuesta virológica,
más EAs y más muertes que los pacientes compensados.
• Se debe evaluar cuidadosamente el tratamiento de los pacientes
con MELD≥ 18, debido a la alta tasa de mortalidad durante o en
las 12 primeras semanas después del tratamiento*.

Resumen
- La eliminación del VHC se asocia a una desaparición/mejoría de las manifestaciones extrahepáticas. Los pacientes con manifestaciones extrahepáticas graves deben ser tratados con independencia del estadio de fibrosis. - Las comorbilidades y tratamientos concomitantes con interacción potencial con los ADDs son muy frecuentes en pacientes con hepatitis C y deben ser tenidas en cuenta en las decisiones terapéuticas
- La existencia de varias alternativas terapéuticas para la hepatitis C con diferentes vías de metabolización/eliminación permiten el tratamiento de la mayoría de los pacientes, incluyendo aquellos con insuficiencia hepática o renal grave
