The introduction of intensive care supports caused a biological revolution: not only have previously unknown clinical conditions (brain death, vegetative state) appeared, but above all manipulating the process of dying has become possible: for the first time in the history of mankind, failing breathing can be supported, and a stopping heart can be restarted. This possibility of intervention calls for responsibility, as interfering in the process of dying is not always in the interest of the dying patient. David Callahan nos señala que uno de los dos principales objetivos de la medicina del siglo XXI es ayudar a las personas a morir en paz. La novedad fundamental del texto que traducimos es haber podido formular unas nuevas prioridades en la práctica de la medicina. Los fines de la medicina, a finales del siglo XX, deben ser algo más que la curación de la enfermedad y el alargamiento de la vida. Han de poner un énfasis especial en aspectos como la prevención de las enfermedades, la paliación del dolor y el sufrimiento, han de situar al mismo nivel el curar y el cuidar, y advertir contra la tentación de prolongar la vida indebidamente. In Spain, elective nontherapeutic Intensive Care to facilitate Organ Donation (ICOD) is defined as: • the initiation or continuation of intensive care measures (eg, admission to the ICU, respiratory support, hemodynamic support); • in patients with a DBI (imminent risk of death of a neurologic cause) in whom intensive care with a therapeutic purpose has been deemed futile; • who are considered possible organ donors (because BD is likely to occur within a short period of time and there are no apparent medical contraindications to organ donation); • to incorporate the option of DBD into their end-of-life care plans. ICOD may help to increase the pool of potential DBD donors and to offer more patients the chance of donating their organs upon their death. The first reference to the practice of admitting patients with a DBI and imminent risk of death into the ICU to facilitate organ donation dates from 1990.19 The protocol of the Royal Devon and Exeter Hospital devised the intubation, mechanical ventilation, and maintenance of these patients until their evolution to BD, to then activate the DBD process. The authors justified the protocol on the basis of the low donation rates described in the United Kingdom at the time and the high mortality on the waiting list. Since then, the practice has been named elective nontherapeutic ventilation in the English-speaking literature. In Spain, the term ICOD is preferred to stress that care for the dying patient remains paramount, to clarify that other intensive care measures beyond elective ventilation are required to make DBD possible and to be inclusive of other clinical scenarios besides that described in the Exeter protocol.
