MECANISMOS DE FALLO DEL TRATAMIENTO
● Proteinas del virus C pueden antagonizar la
actividad antiviral del interferon alfa , aunque no
están bien definidas.
● Factores geneticos también se han implicado en
el mecanismo de no respuesta. Se han
identificado varios genes en los paciente no
respondedores.
● La resistencia a la insulina induce resistencia al
interferón.
Estudios genomicos HCV :USP-18
● Analizando los perfiles de expresión genetica en tejido hepático de pacientes con hepatitis C antes del tratamiento.
● La expresión diferenciada de 8 genes humanos predice la respuesta al tratamiento en el 90% de pacientes.
● Uno de estos genes, USP18 (Ubiquitin-specific protease 18), está presente en gran cantidad en pacientes que no responden a la terapia.

● Disminuyendo los niveles de USP18 mejora la actividad antiviral del interferón frente al virus C 30 veces .
● La inhibición de USP18 provoca cambios celulares que mejoran la sensibilidad al interferon.

CONCLUSIONES (1)
• Entre los pacientes no respondedores se engloban perfiles de pacientes con muy diferentes posibilidades de curación en el retratamiento. Las principales características son: – el tipo de tratamiento (óptimo o no)
– el tipo de respuesta (NR vs. RR),
– Las características basales del paciente (Fibrosis, síndrome metabólico) y del virus (carga viral y genotipo).
CONCLUSIONES (2)• El tratamiento con peginterferón y ribavirina está
indicado en todos los pacientes con tratamiento subóptimo.
• En pacientes no respondedores, el análisis de la carga viral en la semana 12 predice la posibilidad de respuesta y permite la suspensión del tratamiento en caso de positividad.
• En pacientes con ARN VHC negativo en semana 12 se debe recomendar tratamiento durante 72 semanas.
CONCLUSIONES (3)
El tratamiento de mantenimiento
podría estar justificado en pacientes
con enfermedad severa, si produce
respuesta bioquimica (ALT<2N),pero
hay que esperar resultados EPIC y
HALTC.
